This vintage (but new) bell pull fitting style is made from solid metal and features a rod style fitting behind. The width quoted is the width of material which can be folded at the back of the fitting, not the overal width of the bell pull itself.

Pieces can often have a small, shorter strip fitted in cases where the fabric of the bell pull is slightly wider than the fitting. In this case, it may be possible to attach fabric up to 1cm wider (1/2 cm each side). This depends on the fabric in the bell pull and the 'look' that you wish to achieve.
